New Patients
Every new patient must complete a registration form. When you are booked as new patient, you will receive a text message from Hotdoc which includes a link to an online registration form. If you do not receive the link, please use the link below to complete the registration form.

Health Services is open to enrolled Curtin University students and current Curtin University staff only.
If you are no longer a student or staff member, please speak to the GP at Health Service to discuss transitioning your medical care to a medical professional within the community.
Please visit Transfer of Medical Records if you would like your records transferred to another medical centre.
